Dear Patients and Families,

 

It is with mixed emotions that we announce the closure of our medical practice, Lake Pointe Pediatric Associates. Dr. Wieland, Dr. London, Misti Gent, NP, and all of our care team have been honored to provide the health care needs of our patients over the years. Knowing many of of our families for generations makes this exceptionally difficult. For those who are newer to our practice, we regret not having time to get to know you better.

 

We will continue to provide full pediatric care up until the date of our closure on January 23, 2026. If your child is due for a well child visit or an ADHD follow up appointment, we recommend that you schedule as soon as possible as appointments will be limited.

 

If you are not acquainted with another pediatric or family medicine physician, you may use your insurance provider’s directory, check with a local hospital’s physician referral service or check with the local county medical societies to find a new physician.

 

• Patients seen in our practice from 2016 through current may access their electronic health records, including all visits at our clinic, through MyChart. Please call our office to request assistance accessing MyChart if you have not yet signed up for this service.

• Patients who have not been seen since 2016 may request their records in writing and email the request to lppa6900@verizon.net or by fax at 972-475-5708.

• Requests received after March 13, 2026 will be processed by our Custodian of Medical Records, Clary Document Management. If you have previously signed up for MyChart, you can continue to access your charts directly.

 

 

In the event of an emergency after our closing, please call 911 or visit the nearest emergency room.
